Cela influence consid\u00e9rablement une \u00e9ventuelle p\u00e9nurie, mais aussi un exc\u00e8s de soins, contribue \u00e0 l'am\u00e9lioration de la qualit\u00e9 et aide \u00e0 \u00e9conomiser des co\u00fbts. La strat\u00e9gie a heureusement \u00e9t\u00e9 int\u00e9gr\u00e9e dans la planification globale de la politique de sant\u00e9 du canton. Pour que ce mod\u00e8le de soins ne soit pas seulement pr\u00e9sent\u00e9 comme une id\u00e9e futuriste, mais qu'il puisse \u00eatre mis en \u0153uvre de mani\u00e8re pratique, les bases suivantes sont n\u00e9cessaires :\u00a0<\/p><ul><li>Les donn\u00e9es des patients doivent \u00eatre disponibles sous forme \u00e9lectronique,\u00a0<\/li><li>un financement aussi uniforme que possible des diff\u00e9rentes prestations et la pr\u00e9vention de mauvaises incitations financi\u00e8res,\u00a0<\/li><li>\u00e9change professionnel d'informations,\u00a0<\/li><li>Soutien du mod\u00e8le de la part de la Conf\u00e9d\u00e9ration, du canton et des assureurs-maladie,\u00a0<\/li><li>R\u00e9duction des obstacles bureaucratiques,\u00a0<\/li><li>l'\u00e9change de connaissances et d'exp\u00e9riences,\u00a0<\/li><li>des accords de coop\u00e9ration transparents,\u00a0<\/li><li>l'optimisation permanente de la collaboration et de la communication, et bien plus encore.\u00a0<\/li><\/ul><p>\u00a0<\/p><p>Il faut en outre rendre visibles et reconna\u00eetre les mod\u00e8les de soins en r\u00e9seau qui fonctionnent avec succ\u00e8s depuis des ann\u00e9es.
