Le mod\u00e8le aide les patients \u00e0 faire des choix de sant\u00e9 libres et inform\u00e9s et \u00e0 orienter les \u00e9quipes de soins vers les besoins de leur propre projet de vie. Les patients contribuent ainsi \u00e0 l'am\u00e9lioration des soins de sant\u00e9 et de maladie, \u00e0 la formation des professionnels, \u00e0 la recherche et \u00e0 leur propre situation de sant\u00e9 et de maladie ainsi qu'\u00e0 leur satisfaction dans la vie.\u00a0<\/p><p>Nous avons discut\u00e9 de ce mod\u00e8le avec diff\u00e9rents repr\u00e9sentants, mais aussi avec tous les participants et les repr\u00e9sentations de patients en s\u00e9ance pl\u00e9ni\u00e8re, avant de nous r\u00e9partir en ateliers l'apr\u00e8s-midi.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t<div class=\"elementor-column elementor-col-50 elementor-top-column elementor-element
