Mais en r\u00e9alit\u00e9, il faut une nouvelle pouss\u00e9e de travail de pionnier pour faire avancer la coop\u00e9ration et la coordination. Une chose est s\u00fbre : le syst\u00e8me de sant\u00e9 est confront\u00e9 \u00e0 des d\u00e9fis majeurs. Actuellement encore fortement ax\u00e9 sur les soins m\u00e9dicaux aigus, le syst\u00e8me de sant\u00e9 est de plus en plus confront\u00e9 \u00e0 des pathologies chroniques et complexes. Cette complexit\u00e9 est li\u00e9e \u00e0 l'\u00e9volution d\u00e9mographique et \u00e9pid\u00e9miologique globale, \u00e0 l'interaction des caract\u00e9ristiques bio-psycho-sociales des personnes concern\u00e9es, mais aussi aux caract\u00e9ristiques du syst\u00e8me de sant\u00e9. Il ne s'agit donc plus seulement de coordonner la gestion de la maladie, mais surtout de g\u00e9rer l'ensemble des besoins bio-psycho-sociaux et environnementaux, et ce en \u00e9troite collaboration avec les acteurs concern\u00e9s : les patients, les proches et les professionnels de la sant\u00e9 et du social.
