Il s'agit d'une plate-forme d'information pour les cabinets m\u00e9dicaux, qui s'adresse \u00e0 l'ensemble de l'\u00e9quipe du cabinet, aux m\u00e9decins et aux assistantes m\u00e9dicales gr\u00e2ce \u00e0 des outils, des formations modulaires et des \u00e9v\u00e9nements d'information r\u00e9gionaux. En ce qui concerne les maladies non transmissibles, PEPra doit faciliter la mise en \u0153uvre de la pr\u00e9vention dans la pratique quotidienne. Il a \u00e9t\u00e9 \u00e9labor\u00e9 sur la base de donn\u00e9es probantes, il est centr\u00e9 sur le patient et adapt\u00e9 \u00e0 la pratique.<\/p><p>L'une des raisons d'\u00eatre de PEPra est la pr\u00e9valence \u00e9lev\u00e9e des maladies non transmissibles en Suisse :<\/p><ul><li class=\"p1\">Diab\u00e8te, cancer, maladies cardiovasculaires (environ 2,2 millions de personnes).<\/li><li class=\"p1\">D\u00e9pression : diagnostiqu\u00e9e chez 5% de la population (env. 400'000)<\/li><li class=\"p1\">Addictions : un nombre \u00e9lev\u00e9 de cas non recens\u00e9s, un d\u00e9fi majeur pour le syst\u00e8me de sant\u00e9.<\/li><\/ul><p>\u00a0<\/p><p>Dans sa pr\u00e9sentation, Salom\u00e9 Steinle, responsable du projet PEPra, souligne le r\u00f4le central des m\u00e9decins de premier recours dans la pr\u00e9vention. \"Ils sont un point de contact important pour les patients. L'\u00e9quipe du cabinet jouit de leur confiance. Ce lien direct est une ressource pr\u00e9cieuse pour la pr\u00e9vention\". Le retour sur investissement de la pr\u00e9vention est indiscutable. La question est plut\u00f4t de savoir comment l'utiliser, l'organiser et la financer de mani\u00e8re optimale.<\/p><p>Chewith Baumberger, sp\u00e9cialiste MPA \u00e0 la FMH, souligne l'orientation de base de PEPra : \"Du cabinet pour le cabinet\". L'attitude de l'ensemble de l'\u00e9quipe du cabinet est d\u00e9terminante. Mais il souligne \u00e9galement les obstacles \u00e0 la pr\u00e9vention. Bien que beaucoup de choses soient faites syst\u00e9matiquement (d\u00e9pistage, vaccinations, m\u00e9decin scolaire, etc.), la pr\u00e9vention se fait majoritairement sur initiative individuelle. Les programmes sont encore trop ax\u00e9s sur les risques et non sur la population. A cela s'ajoute le fait que le comportement des patients est difficilement influen\u00e7able et souvent contradictoire. Cela conduit souvent \u00e0 des situations absurdes. Liesch cite l'exemple du fumeur qui fait traiter son cholest\u00e9rol, mais qui continue \u00e0 fumer et ne fait pas assez d'exercice.<\/p><p>\u00c9valuer le comportement des patients, c'est un sujet d\u00e9licat, explique Christian Frei de SWICA. O\u00f9 sont les limites ? Finalement, tous les m\u00e9decins n'accorderaient pas non plus la m\u00eame importance \u00e0 la pr\u00e9vention. En ce qui concerne le financement, il estime que l'assurance de base devrait prendre en charge la pr\u00e9vention bas\u00e9e sur les preuves. Mais le d\u00e9fi reste de savoir comment atteindre le groupe cible.
