Nous nous concentrerons sur la collaboration entre le secteur de la sant\u00e9 et le secteur social.<\/p><p>En effet, la sant\u00e9 r\u00e9sulte de l'interaction entre le soutien m\u00e9dical, social et soci\u00e9tal. Mais la r\u00e9alit\u00e9 le montre : Entre le syst\u00e8me de sant\u00e9 et le syst\u00e8me social, il existe des points d'intersection ou de rupture qui compliquent la prise en charge. Dans le format \"feu de camp\" du congr\u00e8s BMC, nous discuterons avec vous de la mani\u00e8re dont la coordination, les r\u00e9seaux et la collaboration interprofessionnelle peuvent combler ces lacunes.
