L'article d\u00e9crit huit initiatives et analyse leurs caract\u00e9ristiques, leurs formes d'int\u00e9gration et les d\u00e9fis qu'elles posent au quotidien. L'objectif est de formuler des recommandations d'action pour des soins durables.<\/p><p>\u00a0<\/p><p><strong> Crit\u00e8res de s\u00e9lection des initiatives de soins :<\/strong><br \/>Les huit initiatives s\u00e9lectionn\u00e9es devaient remplir les conditions suivantes : Elles devaient (1) \u00eatre actives dans le domaine de la m\u00e9decine ambulatoire de base ou communautaire, (2) disposer d'une collaboration interprofessionnelle allant au-del\u00e0 de la collaboration classique entre m\u00e9decins de famille et assistants m\u00e9dicaux, (3) pr\u00e9senter des accords de coop\u00e9ration formels ou informels avec des partenaires externes et (4) provenir aussi bien de Suisse al\u00e9manique que de Suisse romande. Les initiatives destin\u00e9es \u00e0 des groupes cibles sp\u00e9cifiques n'ont pas \u00e9t\u00e9 prises en compte.<\/p><p>\u00a0<\/p><p><strong> Description des trois mod\u00e8les de soins :<\/strong><br \/>Les initiatives ont \u00e9t\u00e9 class\u00e9es en trois grands types :<\/p><ul><li><strong>Type A : Cabinets de m\u00e9decine g\u00e9n\u00e9rale multiprofessionnels ind\u00e9pendants<\/strong><br \/>Petits cabinets priv\u00e9s avec une coop\u00e9ration \u00e9troite entre les m\u00e9decins de famille et les infirmi\u00e8res de pratique avanc\u00e9e (IPA). Ils offrent des soins centr\u00e9s sur le patient et coordonnent les cas complexes. Malgr\u00e9 leur grande flexibilit\u00e9, ils ne sont gu\u00e8re viables \u00e9conomiquement dans le syst\u00e8me tarifaire actuel.<\/li><li><strong>Type B : Centres de sant\u00e9 multiprofessionnels dans des structures de groupe<\/strong><br \/>Ces structures urbaines sont int\u00e9gr\u00e9es dans des r\u00e9seaux ou des groupes. Ils offrent \u00e0 leurs collaborateurs des conditions d'emploi attrayantes et disposent de moyens financiers pour promouvoir la coordination. Cependant, l'int\u00e9gration est souvent bas\u00e9e sur le leadership m\u00e9dical, ce qui limite l'autonomie de nouveaux r\u00f4les (par exemple, les APN).<\/li><li><strong>Type C : Syst\u00e8mes de soins int\u00e9gr\u00e9s r\u00e9gionaux<\/strong><br \/>Regroupement de plusieurs institutions (h\u00f4pitaux, maisons de soins, Spitex, cabinets m\u00e9dicaux) dans des r\u00e9gions structurellement faibles. Ils visent \u00e0 fournir des soins intersectoriels complets.
