Elle a d\u00e9crit la situation initiale en ces termes : \"La demande de soutien et de prise en charge \u00e0 domicile augmente, alors que le personnel qualifi\u00e9 et les structures durables font d\u00e9faut. La prise en charge \u00e0 domicile est encore une affaire priv\u00e9e\". Le d\u00e9fi : une bonne prise en charge pour tous gr\u00e2ce \u00e0 une organisation socialement durable. Il s'agit d'une \"responsabilit\u00e9 et d'une participation cibl\u00e9es et r\u00e9parties sur de nombreuses \u00e9paules\", une \u0153uvre commune qui conduit \u00e0 une culture de la sollicitude. Elle souligne : \"Ceci est con\u00e7u comme un projet de soci\u00e9t\u00e9, une utopie\". Il \u00e9tait important d'impliquer les acteurs locaux d\u00e8s le d\u00e9but, dans la mesure du possible. Ainsi, la situation et les besoins de la population et des professionnels ont d'abord \u00e9t\u00e9 \u00e9valu\u00e9s par le biais d'\u00e9v\u00e9nements, d'interviews individuelles, d'ateliers, etc. Le service d'aide et de soins \u00e0 domicile a \u00e9t\u00e9 retenu comme partenaire. Il s'est rapidement av\u00e9r\u00e9 : Le grand d\u00e9fi pour de nombreuses personnes est d'accepter de l'aide. C'est plus facile quand on se conna\u00eet et qu'on entretient des relations. Lors de la mise en \u0153uvre (quatre communaut\u00e9s d'entraide), il a \u00e9t\u00e9 frappant de constater qu'il n'y avait pas d'autre solution : Des communaut\u00e9s aux pr\u00e9occupations partag\u00e9es ont \u00e9t\u00e9 form\u00e9es et le sentiment d'appartenance, le soutien mutuel, la participation de la population sur un pied d'\u00e9galit\u00e9 ainsi que la collaboration interprofessionnelle ont \u00e9t\u00e9 renforc\u00e9s. Des bases importantes ont ainsi \u00e9t\u00e9 cr\u00e9\u00e9es pour renforcer la capacit\u00e9 de prise en charge locale. En revanche, il n'a pas (encore) \u00e9t\u00e9 possible d'am\u00e9liorer directement la prise en charge des personnes ayant un grand besoin de soutien \u00e0 domicile. Et les participants issus de la population \u00e9taient en majorit\u00e9 des personnes qui s'engagent \u00e9galement ailleurs. Le travail de fond prend beaucoup de temps. Mais la grande force d'int\u00e9gration de la communaut\u00e9 conduit rapidement \u00e0 de petits succ\u00e8s. Par exemple, il devient alors plus facile d'accepter de l'aide. Ici, des appartements pour personnes \u00e2g\u00e9es, un h\u00f4pital et un \u00e9tablissement m\u00e9dico-social sont r\u00e9unis sous un m\u00eame organisme responsable. Les tendances sont les m\u00eames dans les soins de longue dur\u00e9e : l'\u00e2ge moyen des locataires, la multimobidit\u00e9 et les besoins en soins augmentent, alors que le financement et le personnel qualifi\u00e9 font d\u00e9faut. La demande de soins ambulatoires de longue dur\u00e9e augmente, alors qu'elle diminue dans le secteur stationnaire. Les attentes en mati\u00e8re de soins et de qualit\u00e9 sont \u00e9lev\u00e9es.
