Projet de recherche \"Health2040\" de l'Universit\u00e9 de Lucerne et Interface Etudes politiques Recherche Conseil SA.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t<div class=\"elementor-element elementor-element-062316f e-flex e-con-boxed e-con e-parent\" data-id=\"062316f\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-76acd7a elementor-widget elementor-widget-text-editor\" data-id=\"76acd7a\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>En Suisse, les soins ambulatoires de base sont confront\u00e9s \u00e0 de nombreux d\u00e9fis, notamment le vieillissement d\u00e9mographique, la proportion croissante de personnes souffrant de contraintes physiques, psychiques et sociales multiples, l'individualisation ou la pluralisation croissante des parcours de vie, l'\u00e9volution technologique, la pression sur les co\u00fbts et la p\u00e9nurie de personnel qualifi\u00e9. De nombreuses initiatives et projets de recherche ont relev\u00e9 ces d\u00e9fis et visent \u00e0 transformer les structures de soins. Parmi ces projets, on trouve notamment l'\"Agenda soins de base\" annonc\u00e9 par la conseill\u00e8re f\u00e9d\u00e9rale Baume-Schneider en f\u00e9vrier 2024.<\/p><p>Le projet de recherche \"Health2040\" de l'Universit\u00e9 de Lucerne et d'Interface Politikstudien Forschung Beratung AG vise lui aussi \u00e0 faire avancer la transformation des structures de soins ambulatoires, en s'appuyant sur les initiatives existantes, en s'appuyant sur la science et en co-cr\u00e9ant avec les parties prenantes concern\u00e9es. Le pr\u00e9sent plan d'action r\u00e9sume les r\u00e9sultats de ce processus de co-cr\u00e9ation, auquel plus de 50 personnes issues du secteur de la sant\u00e9, du secteur social et de la soci\u00e9t\u00e9 civile ont apport\u00e9 leur expertise. Il pr\u00e9sente le \"r\u00e9seau de sant\u00e9 pour tous - per tutti\" comme la vision d'une prise en charge porteuse d'avenir, identifie les facteurs de r\u00e9ussite et les mesures permettant de r\u00e9aliser cette vision et renvoie \u00e0 des exemples pratiques.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<section
