Pour ce faire, un professionnel de la sant\u00e9, en g\u00e9n\u00e9ral un m\u00e9decin g\u00e9n\u00e9raliste, oriente les patient.e.s vers un \"link worker\". Le professionnel de la sant\u00e9 \u00e9tablit \u00e0 cet effet une ordonnance formelle ou \u00e9tablit directement le contact avec un link worker.<\/p><p class=\"translation-block\">Les link workers sont un nouveau groupe professionnel apparu en Grande-Bretagne, qui travaille comme conseiller et comme interm\u00e9diaire entre le secteur de la sant\u00e9 et le secteur social. En discutant avec les patients qui leur sont adress\u00e9s, les link workers identifient leurs besoins et leurs int\u00e9r\u00eats personnels et \u00e9laborent avec eux un plan d'action. Ce plan comprend essentiellement la mise en relation avec diff\u00e9rentes offres, par exemple le jardinage en commun, des cours de cuisine ou une offre de conseil sp\u00e9cialis\u00e9e. En r\u00e8gle g\u00e9n\u00e9rale, plusieurs rendez-vous sont n\u00e9cessaires pour \u00e9tablir une relation.
