Alors que la coordination fonctionne bien \u00e0 l'h\u00f4pital, les patients* sont souvent livr\u00e9s \u00e0 eux-m\u00eames lors des soins ambulatoires qui suivent, notamment en ce qui concerne le suivi psychosocial. En novembre 2023, le Centre suisse des parapl\u00e9giques de Nottwil (CSP) est devenu le<br \/>Symposium \"Construire des ponts : Une collaboration r\u00e9ussie entre le case management \"r\u00e9f\u00e9rents\" et le service social du CSP\".
