Les points de r\u00e9f\u00e9rence centraux sont la d\u00e9limitation et l'interaction des trois formes de coop\u00e9ration <strong>Assistance, d\u00e9l\u00e9gation et substitution<\/strong>. Alors que l'assistance est comprise comme un soutien purement m\u00e9dical sous la responsabilit\u00e9 directe du m\u00e9decin, la d\u00e9l\u00e9gation d\u00e9crit le transfert d'activit\u00e9s ou de t\u00e2ches m\u00e9dicales d\u00e9finies \u00e0 des sp\u00e9cialistes qualifi\u00e9s non m\u00e9decins. La responsabilit\u00e9 globale reste toujours du ressort du m\u00e9decin qui d\u00e9l\u00e8gue. La substitution, en revanche, d\u00e9signe le transfert durable d'activit\u00e9s m\u00e9dicales dans un domaine de responsabilit\u00e9 autonome, ancr\u00e9 dans la loi, d'un autre groupe professionnel et pr\u00e9suppose les bases juridiques correspondantes.<\/p><p>L'article montre que jusqu'\u00e0 pr\u00e9sent, dans les soins de base suisses, c'est surtout le <strong>Mod\u00e8le de d\u00e9l\u00e9gation<\/strong> a \u00e9t\u00e9 \u00e9tabli. Celui-ci permet de d\u00e9l\u00e9guer des prestations m\u00e9dicales en toute s\u00e9curit\u00e9 juridique, pour autant que des exigences claires soient respect\u00e9es en mati\u00e8re de s\u00e9lection, d'instruction et de surveillance des professionnels d\u00e9l\u00e9gu\u00e9s. Les prestations pouvant \u00eatre d\u00e9l\u00e9gu\u00e9es peuvent \u00eatre soutenues et encourag\u00e9es sur le plan qualitatif par des fili\u00e8res de traitement standardisables et fond\u00e9es sur des preuves.<\/p><p>A l'exemple de <strong>medbase<\/strong> medbase int\u00e8gre syst\u00e9matiquement les infirmi\u00e8res praticiennes dans le mod\u00e8le de d\u00e9l\u00e9gation et a \u00e9tabli \u00e0 cet effet des directives bas\u00e9es sur l'\u00e9vidence, des processus d'instruction structur\u00e9s ainsi que des formes obligatoires de consultation et de supervision professionnelles.
